Program: Organization Information Form

This form is designed to collect information about community organizing groups across the United States. The information you provide here is used in several ways:

  • If you are a community organizing group, we will contact you with information about grants or training programs that may interest your organization.
  • This form is also used to gather the necessary information about your organization to see if your work aligns with some of PTP's specific program offerings, such as our grants program..
  • We use it to build our understanding of the scope of organizing in the U.S. We will be using it to build a public database that will be available on the web.

Suggestions and Instructions for Completing this Page:

  • You may want to prepare your answers in a different program and then cut and paste them onto this screen.
  • This page is pretty long. Make sure you scroll down to the bottom to see all the fields.
  • Please complete the entire form. Don't press Enter, instead, click on the SUBMIT QUERY button at the bottom of the page when you are finished. Once you send the form to us, you will go to a confirmation page in your browser and you will receive an email sent to the address that you give us.
  • Hint: if you get to the confirmation page before you are finished entering your information, just go press your browser's back button to continue from where you left off.

We may publish a directory of community-led organizing groups in the United States. If so, we will use the information that you submit on this page for that purpose. However, we will NOT be publishing individual contact information in the directory without checking with you first.
